When requesting Disclosure, Etc., please download the following request form (A), fill in the required information, and include an identity confirmation document (B), and mail it to 1. above.

(4) Fees and their collection method

1,500 yen per request for disclosure and notification of purpose of use. Please include a fixed-amount postal money order of 1,500 yen (issued by the post office) in your request document. No fee is required for requests for any correction or suspension of use.

*The postage fee for mailing it to the Company and the issuance fee for the fixed-amount postal money order must be borne by you.

*If there is a shortage of fees or if the fees were not included, we will contact you accordingly, but if there is no payment within the prescribed period, we will treat it as if there was no request for disclosure.

(5) Method of responding to requests for Disclosure, Etc.

We will send you a reply by mail to the notification address that you have entered in your personal information disclosure, etc. request form.

(6) "Purpose of use" of the personal information acquired in connection with the request for Disclosure, Etc.

Personal information acquired in connection with the request for Disclosure, Etc. shall be handled only to the extent necessary for the request for Disclosure, Etc.

(7) No disclosure of retained personal data

Disclosure is not made in the following cases. A prescribed fee will still be charged even in the cases of non-disclosure.

- If we cannot verify the requester's identity, for example, when the address on the request form does not match the address on the identity confirmation document

- If we cannot verify the authority of representation in the request made by proxy

- If the subject of the requested disclosure is not "retained personal data"

- If there is a risk of harm to the life, body, assets or other rights of the person concerned or a third party

- If it could significantly impede the proper performance of the Company's work

- If it would violate other laws or regulations
